Comments:
An unannounced complaint inspection was conducted on 9/9/2019, in response to a complaint that was received by the licensing office on 07/29/2019. The inspector was on site from approximately 12:00 PM to 12:45 PM. Documentation was reviewed relating to allegations concerning operational responsibilities. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there were violations found during the inspection regarding staff to children ratio requirements. See the violation notice for more details.

Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Description: Based on observation, the facility did not ensure compliance with ratios of staff to children as required.

Evidence:
1. Based on observation the center did not maintain the ratio of one staff for every four children for children birth to the age of 16 months. 2. In classroom #1, there was one staff person in a classroom with five children birth to 16 months from approximately 12:05 PM to 12:09 PM.
