An unannounced renewal inspection was conducted on Wednesday, October 16, 2019 from approximately 4:05 PM AM until 6:15 PM. The census for today's inspection consisted of 24 children. During the inspection, children were observed working on homework, coloring, reading books, and eating snack. Children's equipment, learning materials, outdoor and indoor space, restrooms, first aid kit and the nonmedical emergency supplies were reviewed. Five children's records and three staff emergency contact information, annual fire and health inspections, liability insurance policy, fire drill, lockdown and shelter in place drill logs, emergency plan, injury prevention plan, daily attendance, and all required postings were reviewed. The provider stated that transportation is provided and the vehicle and transportation documents were reviewed. Medication will be administered per the request of parents and staff with current Medication Administration Training (MAT) will be present when children are in care.
The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and can be found in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-185-270-A Description: Based on observation, the facility did not maintain all areas and equipment of the center, inside and outside, in a clean, safe and operable condition.
Evidence:
1. The bus has two rows of 11 seats each. The seats in rows 2, 3, 4, 5, 6, 7, 8, 9, 10, and 11 on the large bus, used for transportation, are torn exposing the foam.
2. The metal frame in the back of the seats in rows 5,6,7,8 and 9 are exposed.Plan of Correction: Not available online. Contact Inspector for more information.
