Comments:
An announced inspection was conducted at request of licensee for measurements and inspection of new designated program space. Inspection began at 12:10 pm and ended at 1:00 pm. Area of noncompliance are noted on the violation notice. Please contact the Licensing Inspector Christine Mahan with any question (757) 404-0568.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation and staff interviews, the licensee did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the new designated infant/ toddler room there were two containers of disinfectant wipes in an unlocked closet. Both containers were labeled "keep out of reach of children" and at least one of the other statements "caution", "flammable" and "warning".

Plan of Correction: The closet will remain locked when center is in operating hours.

Standard #: 22VAC40-185-550-F
Description: Based on observation and staff interviews, the licensee did not ensure a 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center shall be posted in a visible place at each telephone.

Evidence: In the new designated infant/ toddler room there was a operable telephone and the required emergency numbers were not posted in a visible place.

Plan of Correction: Emergency numbers posted by phone.
