Discussion on background checks for board members.

Comments:
An unannounced renewal inspection was conducted today approximately 10:17 am to 1:30 pm. There were 99 children supervised by 13 direct care staff. The physical plant, 6 staff records, 10 children?s records, children's nutrition, supplies, evacuation drills, injury reports, emergency supplies, and policies were inspected. Medications were on-site today and records were reviewed. The playground and surfacing were inspected. The children were observed in musical movement, group center activity. Diapering changing and hand-washing was observed during the inspection. There were age appropriate materials for all children. Staff-to-child ratios were in compliance.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observations, there were areas and equipment of the center which were not maintained in a clean, safe, and operable condition.
Evidence
1. On the two's playground area, there were sections of the wooden barrier around the fence which were splintering off, that could entangle clothing or snag skin.

Plan of Correction: The piece of wood was fixed during inspection visit.

Standard #: 22VAC40-185-500-A
Description: Based on observation, staff and children's hands were not washed with soap and running water after toileting and any contact with blood, feces or urine. Staff hands were not washed after a staff helped a child use the toilet or a diaper change.
Evidence: Staff #6 did not wash a child's or their hands after a diaper changing. The same staff assisted another child with toileting and staff did not wash their hands after assisting the child.

Plan of Correction: Staff was retrained on 11/15/19 on proper diapering techniques and hand washing instructions/requirements.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not ensure the diapering surface used only for diapering or cleaning children, and it was cleaned with soap and at least room temperature water and sanitized after each use.
Evidence: Staff #6 did not clean the diapering surface with soap and sanitized after changing a child's diaper.

Plan of Correction: Staff member received re-training on how to properly clean and sanitize diaper area.

Standard #: 22VAC40-191-40-D-1-C
Description: Based on record review and interview the center failed to maintain updated background checks as required.
Evidence: There was no record for board member #1 background checks were updated.
2. The administrator stated board member #1 has not had a background check since the initial application.

Plan of Correction: Officer #2 had background checks completed upon Initial Application. Officer lives out of state, has not on site responsibilities and therefore background check wasn't updated.
