Comments:
An unannounced renewal inspection took place on January 2, 2020. There were 22 children in care from ages 7 months old to 7 years old with 5 staff providing care and supervision. The inspector reviewed 5 children's records, 3 board officer records, and all new staff records. This center does not administer medications and does not transport children. Children were observed during the following activities: nap time and quiet activities. The inspector also reviewed emergency drill documentation, first aid kits, and emergency supplies. There were discussions about the renewal process, evacuation route maps, and infant daily records. The inspection was conducted from approximately 12:10 PM to 1:45 PM. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and discussion, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:

The inspector observed a green cot that was torn and detached from a support pole. The inspector observed a child using the cot during nap time.

Plan of Correction: "Upon being notified of the broken cot, all cots were inspected and all damaged (torn) cots were disposed of."

Standard #: 22VAC40-185-420-E-1
Description: Based on observation and daily record review, the center failed to ensure that for each infant, the center shall post a daily record which can be easily accessed by both the parent and the staff working with the child. The record shall contain all the elements as required by the standards.

Evidence:

The forms used by the center as daily records for infants did not contain:

1. Developmental milestones
2. For infants, who are awake and cannot turn over by themselves, the amount of time spent on their stomachs.

Plan of Correction: "Upon being notified of the missing information on the infant daily records, tummy time, along with developmental milestones were added to the infant daily log."
