Comments:
An unannounced complaint investigation was initiated on 11/3/2019 and was concluded on 12/2/2019 regarding infant room care. Based on staff interviews, the preponderance of evidence gathered during the investigation supports the allegations regarding feeding and diapering. The complaint is determined to be valid. Description: Based on staff interviews, the facility failed to follow all diapering procedures as required.
1. Staff did not ensure the immediate safety of a child during diapering. Evidence: Between August 2019 and October 2019, Staff 1 was observed leaving an infant on the changing table unattended. Staff 1 was not within arm's reach to secure Infant A or Infant B during a diaper change when she stepped away to input the information on the iPad.
2. Between August 2019 and October 2019, Staff 1 was observed not wiping an infant's bottom thoroughly after a bowel movement to remove all fecal matter. Staff 1 changed Child C after a bowel movement. Approximately one hour later at the next diaper change, another staff member changed Child C and "had to scrub dried feces off his back". Another day, Child D was changed by Staff 1 after a bowel movement. Another staff member immediately smelled her when she was placed on the floor and saw that "she still had feces on her outfit".

Description: Based on staff interviews, staff did not feed infants on demand or in accordance with parental instructions. Evidence: Between August 2019 and October 2019, a staff member arrived to work at approximately 9:40 a.m. and observed Child E was "in distress". When the staff person asked Staff 1 what was wrong, she stated she didn't know. The second staff tried to console the child, who "was inconsolable". Staff 1 had written on the white board that the next time for Child E was scheduled to get a bottle was at 10:30 a.m. At approximately 10:00 a.m., the staff person checked the iPad to see when the infant had last eaten, which was documented as 5:30 a.m. At the time, Child E was scheduled to get a bottle every three hours.
