An unannounced Monitoring Inspection was conducted today from 10:15am to 12:40pm. There were 116 (Infants to 5 years) directly supervised by 19 staff. The physical plant, 6 staff records, 10 children?s records, 4 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, individual activities and dancing. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Standard #: 22VAC40-185-60-A Description: Based on review, the facility did not ensure that children's records were maintained with the required information.
Evidence: Reviewed 10 children's records and found that Child 3 and Child 7 did not have documentation of work information for one or both parents.Plan of Correction: Child 3 will have documentation tomorrow morning. Will obtain missing information other child tonight.
Standard #: 22VAC40-185-430-B Description: Based on observation, the facility did not ensure that materials and equipment available are age and stage appropriate for the children.
Evidence: Observed in the 1st Steps classroom and the Get Set classroom, a sensory tube that did not have a closed lid allowing the children to access the small objects inside.Plan of Correction: Corrected at the time of the visit.
Standard #: 22VAC40-185-560-G Description: Based on observation, the facility did not ensure that when food is brought from home, the following shall apply: food shall be labeled with the name identifying the owner and the date.
Evidence: Observed lunchboxes in the Pre-K Two, Pre-K One, Preschool 2, Get Set without dates.Plan of Correction: Teachers every morning will place the date on the lunch box and the cook will add dates to all refrigerated food.
