An unannounced complaint inspection was conducted on Monday, August 19, 2019, from 6:56 AM to 9:10 AM in response to a complaint that the licensing department received on August 5, 2019 regarding staffing and supervision. Interviews with staff were completed and documentation was reviewed.
The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Standard #: 22VAC40-185-340-C Complaint related: No Description: Based on a review of staff time cards and interview, the facility did not ensure that during the stated hours of operation one or more children are present, one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age shall be on the premises.
Evidence:
1. The director stated that there was only one teacher between 6:30 AM and approximately 7:00 AM on 8/5/19 and 8/6/19.
2. There were two children in attendance on 8/5/19 and 8/6/19 between 6:35 AM and 7:00 AM.
3. A second staff member arrived at 7:04 AM on 8/5/19 and 7:03 AM on 8/6/19.Plan of Correction: Not available online. Contact Inspector for more information.
