Comments:
An unannounced complaint inspection was conducted in response to a complaint that was received by the licensing department regarding supervision on November 4, 2019. The inspection was initiated on November 6, 2019 at 4:00 PM and completed on November 6, 2019 at 5:30 PM. Interviews with staff were completed, documentation was reviewed and a van was inspected.

The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.

You will need to specify how the deficient practice will be or has been corrected. Just writing the word ?corrected? is not acceptable. Description: Based on a review of policies and procedures and interviews, the center did not ensure that the operational responsibilities of the licensee included, but were not limited to, ensuring that the center's activities, services, and facilities were maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department.

Evidence:
1. Staff stated that 11 children, two to four years of age with two teachers attended the field trip to the pumpkin patch. The center policy states, "For children 4 and under chaperones are strongly encouraged. One adult per four children will be required for all field trips for this age group. Field trips will be cancelled in the event chaperones are not available."
2. One child, two years of age, was left alone and unsupervised by center staff at the playground of the pumpkin patch while the other children and two staff went on a hayride. The center policy states that "staff should always be within sight and sound of all children at all times."

Description: Based on interviews with staff, the licensee did not ensure that when staff are supervising children, they shall always ensure care, protection, and guidance.

Evidence:
1. Staff #1 and staff #2 stated that they were unaware that child #1, two years of age, was not with the group on the hayride and picking pumpkins on 10/21/2019.
2. Child #1, two years of age, was without supervision for approximately 45 minutes.
3. Child #1 was with an employee of the pumpkin patch while the two staff and children went on the hayride and picked pumpkins.

Description: Based on interviews with staff, the center did not ensure children under 10 years of age shall be within actual sight and sound supervision of staff, except that staff need only be able to hear a child who is using the restroom.

Evidence:
1. On October 21, 2019, eleven children and two staff went on a field trip to the pumpkin patch. When staff #1 and staff #2 finished the hayride with the group of children, child #1, two years of age, was bring supervised by an employee of the pumpkin patch.
2. Staff stated that the hayride and pumpkin picking took approximately 45 minutes. Child #1, two years of age, was not with the group on the hayride or picking pumpkins.
3. Staff #1 stated that "all children were counted, getting on hayride."
4. Staff #2 stated that a headcount was done on the hayride.
