63.2(17) License & Registration Procedures

Technical Assistance:
*Discussed checking the menu each week to ensure all food served is listed.
*The exposed landscaping hose and rotting tree stump need to be addressed before they become a safety hazard.

Comments:
An unannounced renewal inspection was conducted on 01/09/2020 from 1:00 pm - 4:20 pm. There were a total of 100 children in care with 13 staff. Three administrative staff were also available. The children in care ranged in age from infant - five years. Children were primary observed during nap. Afternoon snack was observed in the toddler classroom and outdoor play was observed for the preschool classrooms.
Records were reviewed for ten children and eight staff.
Violations were observed in two parts of the CDC standards and are listed on the violation notice.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that children's records contain all of the required elements.

Evidence:
1. The enrollment record for child 1 lacked the following required information:
a. Work number for each parent's place of employment.
b. The address for the second emergency contact.
2. The enrollment record for child 2 lacked a work phone number for one parent's place of employment.

Plan of Correction: The work phone number for the parent of child 2 was obtained during the inspection and documented in the enrollment record.

The parent of child 1 will be asked to provide the missing information at pick-up this afternoon.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on record review and interview, the center failed to obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him.

Evidence:
1. There was not an out-of-state search of the central registry on file from the State of Florida for staff 1.
a. Staff 1 indicated on her sworn statement or affirmation that she had resided in the State of Florida within the past five years.
b. Staff 1 has a hire date of 04/15/2019.
2. Administrative staff confirmed that a central registry check from the State of Florida could not be located.

Plan of Correction: Contacted the State of Florida via email on 1/09/2020 to inquire about the out-of-state background check. If Florida does not respond or have the background check, a new background check will be submitted by January 14, 2020.
