Technical Assistance:
Discussed medication authorization forms with the Director.

Comments:
An unannounced monitoring inspection was conducted today from 12:00pm to 2:45pm with the Director. There were 54 children in care, ranging from five months to five years of age, supervised by 10 staff. Infants were observed playing on the carpet and napping. The older children were observed preparing for nap and napping. The JK class of four- and five-year-olds were observed finishing reading a story before nap time. Five children?s records and five staff records were reviewed and were in compliance. All of the staff have current certification in CPR and First Aid, as well as Daily Health Observation Training. Eight medications and authorization forms were reviewed and the center has eight staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed and were in compliance. The most recent documented evacuation drill was dated 1/6/20 and the most recent documented shelter-in-place drill was dated 11/7/19. The most recent Fire Inspection on file was dated 3/19/19 and the most recent Health Inspection on file was dated 9/5/19. Lunch today was beans and franks, whole wheat bread, applesauce, and milk. Standard #: 22VAC40-185-510-A
Description: Based on review of eight medications for five children, the center did not obtain written parental authorization to give a medication. Evidence: There was an emergency allergy medication on site for Child A, but there was no written authorization from the parent for the center to give the medication.

Plan of Correction: FARE form is on file for the child with the parent and physician signatures for authorization. The additional form was given to the family on 01/14/2020 to have completed by the physician.
