Comments:
An unannounced renewal inspection was conducted today, January 10, 2019 to determine the center's compliance with licensing standards. The inspection was initiated at 10:48 am and concluded at 3:00 pm. Upon the inspector's arrival children throughout the center were observed in a variety of activities. Children in the prekindergarten class reviewed numbers, shapes, colors and emotions. Today was also super hero day and children wore capes and super hero t-shirts. The toddler class danced to music and later played with plastic animals and bugs. Children also looked at books while waiting for lunch. Infants were observed napping, in bouncers and on the floor playing with toys and interacting with staff. Diapering was also observed with hand washing being conducted. Today's lunch consisted of mac and cheese with whole grain noodles, lima beans, peaches and milk. The facility has age appropriate materials and supplies accessible to children in care. The following required information was posted: license, most recent inspection notice, evacuation map/procedures, daily schedules, emergency numbers, menu, attendance sheets and various parent information. The first aid kit and non-medical emergency supplies for the building and van were inspected and found complete. The inspector interacted with staff and children in care.

Six staff records and eight children's records were reviewed.
Medications, authorizations and action plans were reviewed. Staff on site have current MAT training. Staff have current CPR, first aid and daily health observation training.
The most recent evacuation drill was conducted on 12/04/2019. The two required shelter-in-place drills for 2019 were completed on 08/08/2019 and 10/20/2019. A lock down drill was documented on 10/23/2019.

Today's census:
Infant: 8 infants with 2 staff
Toddler: 10 children with 2 staff
Two's: 16 children with 2 staff
Pre-K: 24 children with 3 staff

Violations cited during the licensing period were checked for corrections. There was one repeat violations cited during today's inspection. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of children's records, the center did not obtain documentation of a physical examination before the child's attendance or within one month after attendance.

Evidence:

1. The record of child #8, enrolled 12/05/2017, did not contain documentation of a physical.
2. Administration confirmed documentation of a physical was not in the record for child #8.

Plan of Correction: Per the director: We have reviewed all children's files and ensured filing of all documentation is being filed immediately in the future.

Standard #: 22VAC40-185-160-C
Description: Based on review of staff records, the center did not obtain documentation of a tuberculosis screening for staff at least every two years from the date of the first initial screening or testing.

Evidence:

The most recent tuberculosis screening for staff #6 was dated 08/15/2016, exceeding the two year allowance.

Plan of Correction: Per the director: We have reviewed and updated all staff files and set up calendar reminders for all new staff to notify staff 15 days prior to renewal dates going forward.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff records, the center did not obtain a central registry finding within 30 days of employment or volunteer service.

Evidence:

Staff #3, hired 03/13/2019, had documentation of a central registry dated 05/07/2019, exceeding 30 days from hire.

Plan of Correction: Per the director: We will keep documentation of all returned forms and call to follow up on any reports that do not come in within the first 3 weeks of hire.
