Comments:
A Subsidy Health and Safety inspection was conducted on January 10, 2020 to review supplemental health and safety requirements. The inspection was initiated at 10:48 am and concluded at 3:00 pm. Upon the inspector's arrival children throughout the center were observed in a variety of activities. Children in the prekindergarten class reviewed numbers, shapes, colors and emotions. Today was also super hero day and children wore capes and super hero t-shirts. The toddler class danced to music and later played with plastic animals and bugs. Children also looked at books while waiting for lunch. Infants were observed napping, in bouncers and on the floor playing with toys and interacting with staff. Diapering was also observed with hand washing being conducted. Today's lunch consisted of mac and cheese with whole grain noodles, lima beans, peaches and milk. The facility has age appropriate materials and supplies accessible to children in care. The following required information was posted: license, most recent inspection notice, evacuation map/procedures, daily schedules, emergency numbers, menu, attendance sheets and various parent information. The first aid kit and non-medical emergency supplies for the building and van were inspected and found complete. The inspector interacted with staff and children in care.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Six staff records and eight children's records were reviewed.
Medications, authorizations and action plans were reviewed. Staff on site have current MAT training. Staff have current CPR, first aid and daily health observation training.
The most recent evacuation drill was conducted on 12/04/2019. The two required shelter-in-place drills for 2019 were completed on 08/08/2019 and 10/20/2019. A lock down drill was documented on 10/23/2019.

Infant: 8 infants with 2 staff
Toddler: 10 children with 2 staff
Two's: 16 children with 2 staff
Pre-K: 24 children with 3 staff

Standard #: 22VAC40-665-520-B-11
Description: Based on review of children's records, the center did not obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:

The record of child #8, with a diagnosed food allergy, did not have a written allergy care plan.

Plan of Correction: Per the director: We have contacted and requested updated documentation for all children in our care. We have also reviewed all files to ensure we have proper documentation for all children's records.

Standard #: 22VAC40-665-580-E-1
Description: Based on review of staff records, the center did not ensure staff obtained current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within 90 days of the date of employment.

Evidence:

Staff #3, hired 03/13/2019, had documentation of obtaining cardiopulmonary resuscitation (CPR) dated 08/21/2019, exceeding 90 days from hire.

Plan of Correction: Per the director: We will ensure all new hires are immediately enrolled into First aid/CPR to be completed within the first 30 days of service.
