Standards 22 VAC 40-185-270 (building maintenance) and 22 VAC 40-185-340 (supervision of children) were discussed with the center owner during the inspection.
- Comments:
-
An unannounced monitoring inspection was conducted on 1/15/20 from 1:15pm until 3:10pm. At the time of the tour, there were 16 children in care with 5 staff present. A sample of 5 children's records and 5 staff records were reviewed. Children were observed engaging in free play, playing educational games on the computer, resting quietly during nap time and eating snack. Restroom and hand washing procedures were also observed. Medication, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports and center postings were reviewed. One violation in the area of background checks is identified on the violation notice and was discussed with the center owner during the exit interview.

Standard #: 63.2(17)-1720.1-B-3 Description: Based on record review and interview, the center did not ensure that staff who have lived outside of the state of Virginia within the past 5 years obtain a central registry check from that state.
Evidence:
1. Staff 1 (date of hire: 10/1/19) indicted on her sworn statement or affirmation that she has resided in the state of Ohio within the past 5 years. The record for staff 1 does not contain the results of an out-of-state central registry search for the state of Ohio.
2. The center owner confirmed that the record for staff 1 does not contain the results of an out-of-state central registry search for the state of Ohio.Plan of Correction: The center responded with the following: An out-of-state central registry search request was sent to Ohio for staff 1 upon employment; however, the results were never received. The center director contacted Ohio for follow-up but the follow-up was not documented. On 1/15/20, center director contacted Ohio Professional Registry to request an update. The operator stated they are no longer accepting paper requests and a request should be done through an online portal. An online request was submitted on 1/15/20. The request will take between 30-45 days to be completed.
