Technical Assistance:
Please document lead teacher training for staff who take the 24 hours of lead teacher training to qualify for the position. There must be one lead teacher in each grouping of children with the exception of the first hour of operation, the last hour of operation, and during rest time.

Comments:
An unannounced monitoring inspection was conducted on January 16, 2020 with the center director to determine compliance with the licensing standards. There were 83 children, ranging in age from 4 months to 12 years old present with 14 staff actively supervising. The children were waking from rest time, eating afternoon snack, playing outside, arriving from school, and having free play activities. Snack served today: pepperoni and tortillas. Eight children's files and eight staff files were reviewed - please see Supplemental Summary for details. Four medications for three children were reviewed; four staff are certified in Medication Administration Training. Evacuation drills have been conducted monthly; a shelter in place drill is required twice a year. Please conduct one soon. Standard #: 22VAC40-185-160-A
Description: Based on review of eight staff files, the facility failed to have documentation of a current tuberculosis test or screening for each staff within 21 days of employment. Evidence: Staff 5 (date of hire 5/30/2019) and Staff 8 (date of hire 9/9/2019) did not have documentation of a tuberculosis test or screening within 21 days of employment.

Plan of Correction: Staff will obtain tuberculosis tests or screenings within 21 days of hire.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility failed to keep all hazardous substances such as cleaning materials in a locked place preventing access by children. Evidence: there was a bottle of bleach water and a container of Clorox wipes in an unlocked (red) cabinet in the upstairs school age room there were Clorox wipes in an unlocked cabinet under the sink in the school age bathroom area.

Plan of Correction: Corrected during inspection: the center director locked the hazardous substances in another cabinet.

Description: Based on observation, the facility failed to have toilet paper in each restroom area provided for children. Evidence: there was no toilet paper in two school age bathrooms for boys - one on the first and one on the second level.

Plan of Correction: Corrected during inspection: center director put toilet paper in both bathrooms.

Description: Based on review of the emergency drill log and staff interview, the facility has not conducted two shelter in place drills per year for the most likely to occur scenarios. Evidence: the last documented shelter in place drill was documented 12/14/2018.

Plan of Correction: Center director will conduct a shelter in place drill this month.

Standard #: 22VAC40-191-60-B
Description: Based on review of eight files, the facility failed to have documentation of a completed sworn disclosure statement for each staff prior to employment. Evidence: Staff 6 (date of hire 12/4/2019) did not have documentation of a sworn disclosure statement in the file.

Plan of Correction: Staff 6 will complete a sworn disclosure statement.
