Comments:
An unannounced monitoring inspection was conducted today from 11:45am-2:00pm. There were 41 children directly supervised by 11 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 5 staff records, 6 children?s records, 2 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating lunch which consisted of salisbury steak with gravy, whole grain rice, peas, peaches, and milk, listening to storytime, and napping. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Adequate staff on site are current in CPR/First Aid training, Daily Health Observation Training, and Medication Administration Training. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-130-B
Description: Based on record review, 1 of 6 children's records reviewed did not contain updated immunizations within the last 6 months for a child under the age of two years.

Evidence: Child #4 is under the age of two years did not have updated immunizations on file. The most recent immunizations on file were from 11/26/18.

Plan of Correction: The family was notified by telephone and email on 1/13/2020 and 1/14/2020 to bring updated immunization record. Updated immunization record received on 1/15/2020.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of 5 staff records, the center did not have documentation of following up on an out of state child abuse and neglect central registry search 45 days after the request to search was sent for 1 staff member.

Evidence: Staff #5 (Date of Hire: 9/5/2018) has resided outside the state of Virginia within the last 5 years. Staff #5's record contained documentation that an out of state child abuse and neglect central registry search request was sent on 9/7/2018, however, there was no documentation of the center following up on the request.

Plan of Correction: A follow up phone call was made with the out-of-state agency during the inspection on 01/13/2020. The agency returned the phone call later in the afternoon and indiacted that they could not locate the documentation and to resend the search request. The request was resent on 01/14/2020. All other required background checks such as VA Central Registry, Fingerprints, and another out-of-state background check were on file during the inspection.
