Comments:
The licensing inspector conducted an unannounced complaint inspection on January 7, 2020 in response to a complaint received by our department on November 28, 2019. The inspector was on site from 3:25 PM to 6:10 PM. The allegations of the complaint were regarding care and protection; and parental involvement. Evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Violations were found in areas that were not a part of the original complaint.

Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to the licensing office within 5 business days. Your plan of correction should contain 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on a staff interview, and a review of two staff records; one record did not contain the required information. Evidence: 1. The licensing inspector observed the record for Staff #3 (hire date 9/8/17) did not contain the documentation of education and training hours needed to be qualified as a program leader. During an interview with the licensing inspector, administration stated that Staff #3 is employed as a program leader, but the educational requirement documentation has been misplaced from the staff record. The licensing inspector observed Staff #3 in the role of program leader during the inspection and observed the job title listed as a program leader at hire in the staff record.

Description: Based on review of two staff records and staff interview, the center did not ensure all staff who worked directly with children obtained at least 16 annual hours of staff development activities that are related to child safety and development and the function of the center. Evidence: 1. The licensing inspector observed the record for Staff #3 (hire date 9/8/17) contained no documentation of annual training hours for 2018-2019. 2. Staff #3 reported the annual training had not been completed for 2018-2019.

Description: Based on a record review and staff interview, the facility did not ensure that parents are promptly informed when persistent behavioral problems are identified, and that such notification shall include any disciplinary steps taken in response. Evidence: 1. Staff interviewed identified persistent behavioral problems with Child #2. 2. Staff interviewed reported meetings had occurred with the guardians of Child #2 in reference to persistent behavioral problems. 3. Staff #1 reported there was no documentation available in reference to the identified behavioral problems of Child #2, or in reference to the meetings with the guardian of Child #2.
