Comments:
The inspector conducted an unannounced monitoring inspection on 01/24/2020 from 09:45 am to 2:30 pm. The children were observed eating morning snack, playing in centers, engaging in centers, and playing on the playground. Morning snack consisted of bagels with cream cheese and water. The menu was posted. A formal observation was completed with the twos and the preschool program (3A). Interviews were held with staff throughout the inspection, and the inspector interacted with children when appropriate. All areas of the center were inspected as well as the center's mini buses. Ten children's records and ten staff records were reviewed during this inspection.
The center's first aid kit and emergency supplies were inspected.

The violations from the previous inspections were checked for corrections. There were no repeat violation found during today's inspection.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure that hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1) Bus #2 had two cleaners that were unlocked in the driver's side door.
2) The inspector observed several substances with warnings stating "keep out of the reach of children" in an unlocked closet in the gymnasium.

Plan of Correction: Corrected on site.

Standard #: 22VAC40-185-550-M
Description: Based on record review, the center did not ensure that a complete record of children's injuries were maintained.
Evidence:
1) 3 out of 5 injury records reviewed did not contain documentation of the time the parent was notified.
2) 2 out of 5 injury records did not contain documentation of the date the parent was notified.
3) 2 out of 5 injury records did not contain documentation of the plan to prevent nonoccurence.

Plan of Correction: Director will be the one to review all accident reports prior to days end. Retrain all staff on injury log procedure.
