Discussion on children records, hazardous material and indoor equipment.

Comments:
An unannounced monitoring inspection was conducted today from approximately 10:35 am to 1:00 pm. There were 27 children supervised by 7 direct care staff. The physical plant, 5 staff records, 6 children?s records, children's nutrition, evacuation drills, injury reports, emergency supplies were inspected. No medications were on-site today. The playground and surfacing were inspected. The children were observed in free play, center activities and outside play. There were age appropriate materials for all children. Staff-to-child ratios were in compliance.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on children records review, 1 out 6 records, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #2 (D.O.B 3/13/2018), last documented additional immunization was 7/23/2018.

Plan of Correction: Will continue to review spreadsheet and obtain the required information from the parent.

Standard #: 22VAC40-185-140-A
Description: Based on a review of children records, 3 out of 6 records, each did does not have within one month after attendance a completed physical examination.
Evidence:
1. Child #3 date of enrollment 3/25/2019, did not have a completed physical examination available for review during the inspection.
2. Child #5 date of enrollment 7/17/2019, did not have a completed physical examination available for review during the inspection.
3. Child #6 date of enrollment 11/27/2018, did not have a completed physical examination available for review during the inspection.

Plan of Correction: Will continue to review spreadsheet and obtain required information the parents.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not maintain that hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children.
Evidence: In the Stepping Two's classroom, cleaning supplies were in a unlocked cabinet and accessible to children.

Plan of Correction: Corrected during the visit and placed out of reach on a shelf. I will talk to the staff.
