Comments:
Thank you for your assistance during this unannounced inspection. This-onsite visit was conducted as a result of a complaint that was received by the Valley Licensing Office on January 16, 2020 regarding the allegation of supervision and emergency procedures. The information gathered during this investigation supports emergency procedures, therefore, the complaint is valid.

Please complete your plan of correction within "five business days" and return to the licensing inspector.

Standard #: 22VAC40-185-550-L
Complaint related: Yes
Description: Based on staff and parent interviews, the center failed to notify the parent by the end of day for a minor injury to one child.

Evidence:
1. Staff #1 stated on 01/21/2020, at approximately, 9:51 a.m., the students were outside playing on the playground and a student told her Child #1 was hurt.
2. Child #1 was in the care of Staff #2. Staff #1 told Staff #2 the child was hurt.
3. Staff #2 stated she went to check on Child #1. She asked him what was wrong and he pointed to his back. Child #1 was not crying.
4. Staff #2 got an ice pack and gave it to Child #1.
5. Staff #2 did not notify the parent at pick up the Child #1 had been given an ice pack.

Plan of Correction: Staff will be retrained on incident/accident procedures. A signed form will be placed in their personnel file.

2. Child #1 was in the care of Staff #2. Staff #1 told Staff #2 the child was hurt.
