Reminder that all staff files at the corporate office contain staff's emergency contact information, job title, documentation of annual training and a copy of staffs high school diploma or the equivalent or documentation of higher education.

Five staff files were reviewed at corporate office today from 9: 30 am to 5: 30 pm.
Please submit documentation of the Out of State Central Registry Finding or that you have sent off for the background check for Staff A by 2/7/20.

Standard #: 63.2(17)-1720.1-B-3 Description: Based on a review of five staff files and an interview with corporate office staff, it was determined that one staff did not have out of state child abuse and neglect search request sent within 30th day of employment nor was their any information about the follow-up.
Evidence:
Staff A was hired on 9/11/17 and lived in Missouri and Nebraska in the last five years however there was no information on file regarding the out of state child abuse and neglect background searches.Plan of Correction: We will send off for these background checks.
