Comments:

An unannounced monitoring inspection was conducted today from approximately 9:50am-11:20am. There were 26 children in the direct care and supervision of 7 staff. A sample of 5 children?s records, 5 staff records and 5 minor injury logs were reviewed. The children were observed drawing, engaged with staff, preparing to go outside and being dismissed. Four new staff members have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the administrator. Description:	Based on observation, the staff could not provide proof of current liability insurance. Evidence: The liability insurance on file expired 10/01/2019

Description:	Based on observation, children's records were not treated confidentially. Evidence: Children's files were located in an unlocked cabinet.

Description:	Based on record review, children's records were missing required information. Evidence: 1 of 5 records reviewed were missing required information. 1. Child A was missing the residential and employment information for the father.
Plan of Correction:	Files were corrected with N/A for absent parent information per suggestion of licensing rep.

Description:	Based on record review, staff records were missing required information. Evidence: 1. Staff A -E were missing addresses for their emergency contact. 2. Staff A was missing date of hire. 3. Staff B was missing date of hire. 4. Staff C was missing a degree or transcripts.
