Technical Assistance:
Cribs that have chipped paint on top railing may indicate duration a mobile infant remains in the crib. During last inspection, it was noted an awake infant was in the crib for over 30 minutes. Attendance records are required to accurately record when a child actually arrives and leaves and not have marks that say same each day. First Aid certification will expire in August, 2019

Comments:
An unannounced inspection was conducted from 1030- 1145. The Vendor was home but no children were in care. The vendor stated the one child will arrive in the afternoon and three children had recently stopped receiving care. The home has some supplies, has a complete first aid kit, and vendor has recorded emergency drills but Inspector saw that vendor has an evacuation drill planned for today. Standard #: 22VAC40-665-160-B
Description: Based on record review and interview, it was evident the vendor did not accurately maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs. Evidence: 1. Vendor stated she has only one child enrolled and the other children left recently. 2. Children 2-4 have attendance records that ended in May, 2018. 3. Child #1 was not present at the time of the inspection and the vendor stated she would come in the afternoon. The attednance sheet for today has marks stating she arrived at 7:30 and leaving at 3:30.

Plan of Correction: I will only write when they come in and when they leave.

Standard #: 22VAC40-665-180-2
Description: Based on record review, it was determined the vendor did not have documentation of finger print checks for herself and household member.

Plan of Correction: Inspector requested info from the local Case worker but also requested Vendor to send documentation.

Standard #: 22VAC40-665-240-A
Description: Based on observation, it was determined that parts of the area and equipment used by children is not maintained in a clean and safe manner. Evidence: 1. The Vendor has two cribs that she states is used by the children and both cribs have evident paint chipping and peeling on the top railing of the cribs. 2. Ceiling in the bathroom has evident leak from above with paint and repair needed.

Plan of Correction: I will talk to maintenance about bathroom. I will paint the cribs or change them.
