63.2(17) License & Registration Procedures
An unannounced monitoring inspection was completed on 1/30/2020. There were 26 children present with two staff supervising. Five children's records were reviewed and two staff records had been previously reviewed. No medications were at the center for any children in care.
The inspector arrived at the school at 3:55 pm and departed at 5:30 pm.
The children were observed during computer lab time and during departures.
Standard #: 22VAC40-185-60-A Description: Based on record review, the center failed to ensure that all children's records contained documentation of all required information.
1. The record for child #1 was missing the child's first date of attendance.
2. The record for child #2 was missing the addresses for two of two required emergency contacts for the child. A name, address, and phone number is required for two people to contact if the parents can not be reached.
3. The record for child #3 was missing a work phone number for the mother and father of child #3.
Plan of Correction: The site director will get the information and document it in each child's file.
