Comments:
A Subsidy Health and Safety inspection was conducted on 1/27/20 from 1:45pm - 4:15pm. During the inspection there were 57 children ages four to ten years old in care with nine staff. Children were observed participating in various activities in the gym, and eating snack. Records were reviewed for five children and seven staff during the inspection. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-510-B
Description: Based on observation and interviews, it was determined that the vendor did not ensure that for each group of children, the vendor shall maintain a written record (hard copy) of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
1. The Licensing Inspector reviewed the attendance records for the facility, and the attendance records did not have the departure documented for all children in care.
2. Staff #6 confirmed that the written record of daily attendance being used does not document departure of each child in care as it occurs.

Plan of Correction: The vendor responded: Staff will be re-trained on attendance procedures to ensure that arrival and departure of each child is documented on the attendance sheet. center management will double check the attendance record daily to make sure it is complete.

Standard #: 22VAC40-665-580-B
Description: Based on a review of six staff records, it was determined that the vendor did not ensure that within the first 90 days of employment all staff shall complete the Virginia Preservice Training for Child Care Staff.

Evidence:
1. The record for staff #1 did not include documentation to demonstrate complete of the all staff shall complete the Virginia Preservice Training for Child Care Staff.
2. The record for staff #3 did not include documentation to demonstrate complete of the all staff shall complete the Virginia Preservice Training for Child Care Staff.
3. Staff #6 reviewed the records for staff #1 and staff #3, and was unable to provide any documentation during the inspection to demonstrate completion of the Virginia Preservice Training for Child Care Staff by either staff.

Plan of Correction: The vendor responded: Staff #1 and staff #3 will complete the Virginia Preservice Training for Child Care Staff by 2/28/20. All new staff will complete the Virginia Preservice Training for Child Care Staff within 90 days of employment.
