Comments:
An unannounced monitoring inspection was conducted on Wednesday, December 11, 2019 to determine the center's compliance with licensing standards. The inspector arrived at 2:42 PM and concluded the inspection at 4:45 PM. The census for today's inspection consisted of 24 children. During the inspection, children were napping, washing hands, eating snack, playing with cars and trucks, dancing and completing homework. Children's equipment, learning materials, indoor and outdoor space, restrooms, first aid kit and the nonmedical emergency supplies were reviewed. The provider stated that medication is not administered at this time. The provider stated that transportation is provided. One vehicle and transportation documents were reviewed. The additional vehicle was in use and not at the center during the inspection. Five children's records and five staff records, annual fire and health inspections, fire drill, lockdown drill and shelter in place drill logs, daily attendance, and all required postings were reviewed.

The director was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were found and are identified on the violation notice.

Please complete the ?plan of correction? and ?date to be corrected? for each violation listed on the violation notice and return it to me within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 22VAC40-185-450-A
Description: Based on observation, the center did not ensure that cots used by children during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Evidence:
In classroom #1, one child did not have a bottom cover during the designated rest time.

Description: Based on observation, the center did not ensure that a nonabsorbent surface for diapering or changing shall be used.

Evidence:
In classroom #4, the diaper changing pad had small tears and was no longer nonabsorbent.

Description: Based on observation, the center did not ensure that when diaper cream is used all requirements are met.

Evidence:
1. In classroom #4, a tube of diaper ointment was not labeled with the child's name.
2. Staff stated that the child did not have written parent authorization noting any adverse reactions to the diaper ointment.
