Comments:
An unannounced monitoring inspection was conducted between the hours of 12:00pm and 3:00pm. Upon arrival there were 90 children in care with 12 staff present. A sample of 2 staff record were reviewed. Children were observed during nap. Infants were observed sleeping in cribs, being fed and on the floor interacting with staff.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
In the 2 year-old room there was a bottle of bleach water on the children's cubbies and a bottle of window cleaner on top of the storage cabinet.

Plan of Correction: The following plan of correction was provided by the director during the exit interview:
The chemicals were placed in locked areas during the inspection. The director will remind staff about locking chemicals and will monitor for future compliance.

Standard #: 22VAC40-185-290-3
Description: Based on observation the center did not ensure all electrical outlets had protective covers.
Evidence:
There were 2 electrical outlets in the main hallway that did not have protective covers on them.

Plan of Correction: The following plan of correction was provided by the director during the exit interview:
Protective covers were placed on the electrical outlets during the inspection.

Standard #: 22VAC40-185-440-J
Description: Based on observation the center did not ensure all crib requirements were met.
Evidence: There were two babies observed sleeping in cribs that did not have at least 30" of space on the service side. One crib had a rocking chair blocking the service side and the other crib's service side was blocked by another baby in a exersaucer.

Plan of Correction: The following plan of correction was provided by the director during the exit interview:
The director will discuss crib spacing with all infant staff and will monitor for compliance.
