An unannounced renewal inspection was conducted today between approximately 10:30 am and 12:45pm. The program is a balanced mixed age group with 21 children directly supervised by 2 staff members. Ratios for a balanced mixed age group were in compliance. The physical space, 5 children's records, staff records, attendance, the Emergency Preparedness plan, evacuation drills, injury reports and required posted documents were reviewed. Medications were in compliance. Children were observed in small group activities, interacting with the staff, playing outside on the playground and washing hands to prepare for lunch after outside play. All food is brought from home and lunch bags are properly labeled. Developmentally appropriate materials and activities were available to the children. Evidence: The current immunization records for Child #1 were missing from the file.
Plan of Correction: We will ask for a copy of the records to be sent in right away.
Standard #: 22VAC40-185-140-A Description: Based on record review, 1 out of 5 children's records did not have a physical examination on file.
Evidence: The file for Child #1 was missing the document.
Plan of Correction: We will ask for the form to be sent in right away.
Standard #: 22VAC40-185-420-E-3 Description: Based on record review, staff did not request, at least annually, parent confirmation that the required information in the child's record is up to date.
Evidence: 5 out of 5 children's records did not have documentation showing annual review/updates.
Plan of Correction: Currently we have parents review the files prior to the new school year. We will ask them to sign and date a document to acknowledge the emergency information has been reviewed and updated if needed.
Standard #: 22VAC40-185-550-M Description: Based on record review, 4 out of 5 injury reports did not contain any future action to prevent recurrence of the injury.
Evidence: The space requiring the information was left blank on 4 of the injury reports.
Plan of Correction: We will make sure to fill out the form completely.
