Comments:
An unannounced renewal inspection was conducted on 02/05/2020. There were 27 children, ages 2 years-4 years, and 6 staff members present during the inspection. The inspector reviewed 5 children's records, 5 staff records and 3 manager records during the inspection. The staff stated that there is no medication currently on-site. The children were observed in the following activities: in free choice activities, participating in art projects, during story time, having lunch and during nap time. The inspector reviewed the required policies and procedures and the emergency supplies during the inspection. The inspector discussed the following with program staff: hand washing, restrooms, children's records and nutrition. The inspector arrived for the inspection at 9:45 a.m. and departed at 1:45 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review and discussion with the staff, the facility failed to obtain a TB test/screening at least every two years from the date of the first initial test/screening.

Evidence: The record for Staff #3 contained a TB test/screening dated 12/12/2017. There was not a follow-up test for 2019 in the record. Staff confirmed a follow-up test has not been obtained.

Plan of Correction: Staff #3 made an appointment to get a repeat test during the inspection.

Standard #: 22VAC40-185-270-A
Description: Based on observation and discussion with the staff, the facility failed to ensure that the areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence: The plastic boat playground toy has a crack in it that creates a sharp edge and a pinch point. The crack is accessible to children playing on the toy.

Plan of Correction: The toy will be removed.

Standard #: 22VAC40-185-290-3
Description: REPEAT VIOLATION

Based on observation and discussion with staff, the facility failed to ensure that in areas used by children of preschool age or younger, electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

Evidence:
1. There were 2 uncovered electrical outlets in the 2 year old classroom.
2. There was 1 uncovered electrical outlet in the Pre-K classroom.
3. Children were observed in both of the classroom during the inspection.

Plan of Correction: The outlets will be covered.

Standard #: 22VAC40-185-330-B
Description: Based on observations, the facility failed to ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath and surrounding equipment that requires a resilient surface.

Evidence: The concrete below the resilient surfacing is visible in the fall zone of the small movable plastic piece of playground equipment with the blue slide.

Plan of Correction: Mulch will be added to the playground as soon as the weather allows.. .

Standard #: 22VAC40-185-380-A
Description: Based on observations and discussion with the staff, the facility failed to ensure that there shall be a posted daily schedule for toddles and preschoolers.

Evidence: There was not a posted daily schedule in the Pre-K classroom.

Plan of Correction: A schedule will be printed and posted.

Standard #: 22VAC40-185-560-G
Description: REPEAT VIOLATION

Based on observation and discussion with staff, the facility failed to ensure that when food is brought from home, the container shall be clearly dated and labeled in a way that identifies the owner.

Evidence:
1. There were 2 sippy cups in the small refrigerator of the 2 year old class that were not labeled and dated. Staff #3 verified that the cups were brought from home.
2. There was a ziplock baggie of chips in a cubby of the school-aged room that was not labeled and dated.

Plan of Correction: Food from home will be labeled and dated.
