Comments:
An unannounced monitoring inspection was conducted today from approximately 8am-10am. There were 56 children in the direct care and supervision of 18 staff. A sample of 5 children?s records, 5 staff records, 5 medications and 5 minor injury logs were reviewed. The children were observed eating breakfast, reading, listening to music, learning about colors, and learning about flowers. The children went on a field trip to the firehouse today. Two new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the director. Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review, staff records were missing required information.

Evidence:
Staff A and B were missing the address of their emergency contact.

Plan of Correction: Staff specific files reviewed have been coached on appropriate documentation procedures. Audits of files will be conducted to eliminate missing information.

Standard #: 22VAC40-185-350-E-4
Description: Based on interview and observation, a classroom did not meet the ratio requirements for 3-5 year olds.

Evidence:
1. Room 5 was out of ratio for approximately 15 minutes.
2. There were 11 children, ages 3-5 years old, in the room with one staff.
3. At 8:43 am Staff C stated that an additional staff member will come in at 9 am.

Plan of Correction: Classroom ratio will be monitoring using a data-based check of ratios throughout program operations has been created by the Education Manager and presented for use by all management. The sheet is located at the front desk for access.

Standard #: 22VAC40-185-550-L
Description: Based on record review, a parent was not notified of a minor injury.

Evidence:
A minor injury was documented for Child A on 10/08/2019 and the parent notification area was left blank.

Plan of Correction: Review of incident report procedures has been reviewed with staff/ management. Ongoing audits of incident reports will be implemented by management to eliminate missing information.

Description: Based on record review, minor injury reports reviewed were missing required information.

Evidence:
Five minor injury reports were reviewed and the following was missing.
1. One was missing the future action to prevent injury.
2. Two were missing the date the parent was notified.
3. Two were missing the time the parent was notified.

Plan of Correction: Review of incident report procedures has been reviewed with staff/ management. Ongoing audits of incident reports will be implemented by management to eliminate missing information.
