Violations:
Standard #: 22VAC40-185-80-A
Description: Based on observation and interviews with staff, it was determined that the center is not maintaining an accurate record of children present in each classroom.
Evidence:
In classroom D ( 3 to 6 years of age) the attendance log listed 27 children as being present when initially there 23 children present. Five children left which resulted in 18 children present, however the attendance log still showed 27 children as being present in the classroom.

In classroom E there were 5- kindergartners present however there was no attendance log.

Plan of Correction: I will create an additional attendance form so that the staff can keep an accurate attendance of the children in the classroom at all times.

Standard #: 22VAC40-185-510-J
Description: Based on observation, it was determined that medication was not kept in a locked place.
Evidence:
The Licensing Inspector observed two Epipen's unlocked in a bag on top of a cabinet in Classroom A.

Plan of Correction: The medication was locked during the inspection and I will remind staff to keep all medication in the locked cabinet.
