Comments:
An unannounced inspection was completed on 02/10/2020 from 10:00 A.M.-2:50 P.M. At the time of the inspection 57 children were in care with twelve staff and one administrator present. Six children records, six staff records, three medications and required paperwork, six injury reports, first aid kits, emergency supplies, outdoor play areas, injury prevention plan, staff handbook, playground safety plan, and posted required information were reviewed. Children were observed during infant feeding,diapering, infant interactions, handwashing small group activities, center play, storytime, transition times, lunchtime, behavior guidance, departure of half day students, and naptime. The exceptions to compliance are noted on the violation notice.

Standard #: 22VAC40-185-240-B
Description: Based on a review of the center's written policies and procedures and interviews with administrative staff, staff were not provided in writing with all required information by the end of the first day of supervising children.

Evidence:
1. Documentation provided to staff did not contain the following required information; procedures for supervising a child arriving after scheduled classes or activities including field trips; procedures to confirm absence of a child when arriving from another program or agency, and procedures for identifying where attending children are at all times including field trips.
2.Administrative staff verified the required policies were not included in the written information that employees are provided upon hire.

Plan of Correction: Administration will add the required policies to the staff handbook, and provide updated handbooks to all staff. Staff will sign an acknowledgement page that will be placed in their file.

Standard #: 22VAC40-185-540-E
Description: Based on observation, the facility failed to have a working battery-operated radio.

Evidence:
1. The radio located in the office had batteries, but the batteries did not work. Administration checked with all classrooms to locate a battery operated radio, but one could not be located.
2. Administration checked supplies to see if any extra batteries were available for use. There were no extra batteries on cite.

Plan of Correction: Administration placed an order for batteries and will add to monthly playground check to check all non medical emergency supplies.
