Technical Assistance:
Today we discussed disposable bottles and locking diaper cans

Comments:
An unannounced monitoring inspection was conducted today from 9:00 am ? 12:00 noon. There were 76 children in care ages four months through four years old with 11 staff. There were three administrators present as well. Children and staff records were reviewed. Required postings, annual inspections, emergency evacuation drills, first aid kit and medication were reviewed. A walking of the center inside and outside was completed. Children were observed engaged in teacher and self-directed activities including morning snack, restroom routines, transitions hand washing, and inside and outside play activities. Violations regarding staff records, children's records, physical plant and emergency items were identified and appear on the violation notice. The violations were discussed with the owner and the program director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review and interview the center failed to ensure that each has physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.
Evidence:
1 - The records for child 1 and 2 do not contain documentation of physical examination.
a, Child 1 has a documented first date of attendance as August 27, 2018.
b. Child 2 has a documented first date of attendance as June 17, 2019.
2 - Staff 1 and 2 confirmed that the children do not have physicals on file today.

Plan of Correction: The information will be obtained from the parent.

Standard #: 22VAC40-185-70-A
Description: Based on record review and interview the center failed to ensure that each staff record contains all required information.
Evidence:
1 - The record for staff 3 does not contain documentation of education.
a. Staff 3 has a documented date of hire as October 14, 2019.
2 - Staff 1 and 2 confirmed that documentation of education was not present in the record for staff 3.

Plan of Correction: The information will be obtained and placed in the staff record

Standard #: 22VAC40-185-270-A
Description: Based on observation the center failed to maintain areas in safe and operable condition.
Evidence:
1 - Dangling electrical cords from a radios in the school age room and the toddler/two year old room.
a. The cords were dangling in such a way that they were within reach of children and as such posed a toppling hazard.
2 - There is peeling red paint/plastic casing on one of the tricycles on the playground.
a. The peeling area is within reach of children.
b. The peeling area flaked off easily when touched.

Plan of Correction: All dangling cords were corrected during the inspection. Staff will be reminded and retrained in the importance of ensuring the electrical cords are not dangling within reach of children.
The tricycle with peeling paint will be removed from playground until such time as the peeling paint can be repaired or the tricycle replaced.

Standard #: 22VAC40-185-500-A
Description: Based on observation the center failed to ensure that children wash their hands after eating.
Evidence:
1 - Upon finishing morning snack three children in the preschool room got up from the table and sat on a couch near the meeting carpet. The children did not wash their hands before going to the couch.
2 - Staff 2 confirmed that children did not wash their hands before going to the couch. Staff 2 stated that they all go to the bathroom as a group after everyone had was finished eating.

Plan of Correction: All children will either use a wipe before leaving the table after meals and snacks or will remain at the table until directed to the restroom to wash their hands.

Standard #: 22VAC40-185-570-I
Description: Based on observation and interview the center failed to ensure that a one-day's emergency supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care is maintained at the center.
Evidence:
1 - Staff present in the infant care room stated that they did not know about a one day's supply of disposable bottles, nipples and formula.
2 - Management staff reported one day's emergency supply had been depleted from daily use rather than being saved for emergencies only as was intended.

Plan of Correction: A one day's supply of disposable bottles, nipples and appropriate formula will be maintained in the infant room. All staff will be retrained and reminded that the emergency supply bucket is to be used for true emergencies only.

Standard #: 22VAC40-185-580-C
Description: Based on interview the center failed to ensure that emergency numbers are posted in the vehicles.
Evidence:
Management staff reported that emergency numbers are not posted in vehicles as required.

Plan of Correction: A placard will be placed in the outside pocket of each transportation vehicle and displayed in the vehicle during all transportation.
