Comments:
An unannounced monitoring inspection was conducted between the hours of 4:00 pm and 6:00 pm. At the time of the tour there were 23 children in care. Children were observed transitioning from school to the after-school program, eating snack and interacting with staff. A sample of 3 staff and 3 children's records were reviewed, in addition to 3 medications.

Violations:
Standard #: 22VAC40-185-350-C
Description: Based on observation The center was not in compliance with the ratio requirements when supervising children in regularly mixed age groups.
Evidence:
When the inspector arrived at the center around 4:00 pm there were 23 children, with the youngest child, 3 years-old with 2 staff present until around 4:30 pm The ratio for 3 year-old children is 1 staff per 10 children.
1. The center regularly operates with a group of children of mixed ages ranging 3 - 12 years-old
2. Staff #2 told the inspector the third staff was in the building, however, had not come to the gym area yet.

Plan of Correction: The following corrective action was provided by the director during the exit interview: The director is also employed with the school and is off the clock at 4:00 pm. This is the same time children are departing, and the after-school children meet in the gym. Champions staff also arrive at 4:00pm to set-up in the gym. The director will work with the district manager on a plan to maintain compliance with the ratios during this transition period until the director can join the staff.

Description: Based on observation the center did not ensure all requirements were met when providing meals and snacks.
Evidence: Some children in the program were observed eating pulled pork sandwiches and others were eating corn dog nuggets. The menu for the day stated the pulled pork was to be served. There were no substitutions notes on the menu.

Plan of Correction: The following plan of correction was provided by the director during the exit interview:
This was an oversight. There were left over corn dog nuggets from lunch the day before and were offered. In the future all substituted foods will be indicated on the menu.
