Comments:
An unannounced monitoring inspection was conducted on February 6, 2020, from approximately 10:00 am to 12:20 pm. Upon entrance there were 65 staff were present with 84 children and there were additional staff on site for administrative purposes and program support. Children were observed in center activities, arts and crafts, building activities, signing and writing activities. The sample size of records reviewed included 9 children's and 8 staff records. Areas of non-compliance are identified on the violation notice.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review, facility did not ensure 1 of 8 children's records has a physical within a month of beginning attendance.
Evidence: The record for child #7( start date February 11, 2019) did not have a physical.

Plan of Correction: New management is auditing all children?s records to ensure compliance on or before 3/13/20.. DM will do random audits during school visits for oversight.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the licensee did not ensure the diapering surface shall be used only for diapering or cleaning children, and it shall be cleaned with soap and at least room temperature water and sanitized after each use.

Evidence: In the toddler classroom, there was a clipboard, paper and chalk stored on the diapering surface.

Plan of Correction: All staff in diaper changing classrooms will be retrained on proper diapering and sanitation processes on or before 2/10/20. Management will observe daily for compliance.
