The inspector conducted an inspection based on a complaint received in this office on 11/10/2016. There were 15 children in care with staff supervising. Two staff records and three children's records were reviewed. There was one violation from today's inspection. The inspection began at 9:50 a.m. and ended at 10:50. Description: Based on record review, the center did not obtain documentation of immunization for children in care. Evidence: 1. The inspector reviewed three children's files. Two of three records did not contain immunization records.

Plan of Correction: The staff will get immunization records from the parents and file them in the records.
