Comments:
An unannounced monitoring inspection was conducted on 02/11/2020. There were 10 children, ages 6 years-11 years, and 2 staff members present during the inspection. The inspector reviewed 5 children's records, the on-site staff record requirements for 2 staff members and one medication during the inspection. The children were observed in the following activities: in large motor play in the gym, having an afternoon snack, during homework time and in free choice activities. The inspector reviewed the emergency supplies during the inspection. The inspector discussed table sanitizing agents with the staff. The inspector arrived for the inspection at 3:20 p.m. and departed at 5:15 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Standard #: 22VAC40-185-70-A
Description: Based on discussion with the staff, the facility failed to ensure that the staff record contain all of the elements as required by the standards.

Evidence: There was not information regarding the name, address and telephone number of a person to be notified in an emergency, which shall be kept at the center, available for review for Staff #2.

Plan of Correction: A form was completed during the inspection.

Standard #: 22VAC40-185-500-A
Description: Based on observation and discussion with staff, the facility failed to ensure that children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.

Evidence: The children did not wash their hands with soap and running water or disposable wipes after the afternoon snack.

Plan of Correction: The children will be taken back to the restroom if there are no wipes available.

Standard #: 22VAC40-185-510-C
Description: Based on medication review and discussion with the staff, the facility failed to ensure that the authorization for medication administration includes the duration of the parent's authorization.

Evidence: The medication authorization form for Medication A did not include the parent's duration for medication administration.

Plan of Correction: The parent will be asked to add a duration to the form.

Standard #: 22VAC40-185-560-F
Description: Based on observation and discussion with the staff, the facility failed to ensure that when centers choose to provide meals or snacks the menu listing foods to be served for meals and snacks during the current one-week period shall be dated and posted in a location conspicuous to parents.

Evidence: The menu that was posted was for January 2020.

Plan of Correction: The February 2020 menu was posted during the inspection.

Standard #: 22VAC40-185-560-K
Description: Based on observations and discussion, the facility failed to ensure that tables shall be sanitized before and after each use for feeding.

Evidence: The sanitizing agent (bleach and water) was sprayed on the tables and immediately wiped off before being used for afternoon snack; the manufacturers instructions indicated that the product should sit for 5 minutes before being wiped off. The tables were not sanitized after being used for the afternoon snack.

Plan of Correction: The tables will be sanitized before and after snack.
