Comments:
2/4/2020 - Site visit for renewal. Licensing Inspector arrived at 3:40 and departed at 4:45 to conduct the site visit for the renewal. At the time of the inspection, there were 17 school-age children in care with two staff. Children were observed playing in the gym and eating lunch. There was no medication be administered or stored on site. The sample size was five children's records.

2/12/2020 - Record review for renewal conducted at AlphaBest office. The sample size was three records.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, it was determined that the facility did not ensure that all children had a completed physical on file within 30 days of enrollment.

Evidence:
The record for child #5 (date of enrollment 9/3/19) did not include a physical examination.

Plan of Correction: Center management will work with parent to obtain physical and continue to audit children's records for completeness.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, it was determined that the facility did not ensure that all required elements were present in each child's record.

Evidence:
The record for child #3 did not include an address for one of the two emergency contact persons.

Plan of Correction: Center management will contact the parent to obtain the address and will continue to audit the children's records for completeness.

Standard #: 22VAC40-185-280-B
Description: Based on observation of the physical plant, it was determined that the facility did not ensure that all hazardous substances were stored in a locked location.

Evidence;
The lock on the storage cabinet belonging to the program was broken. There was a bottle of bleach stored on the bottom shelf.

Plan of Correction: New locks will be purchased and used at the facility.
