Comments:
A Subsidy Health and Safety inspection was conducted on 02/11/2020 to review supplemental health and safety requirements. The inspector observed morning snack and activities. Twelve children, ages 19 months and older, were in care with two staff. Five staff records and five children's records were reviewed.
A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Violations:
Standard #: 22VAC40-665-510-B
Description: Based upon observation, the facility has not ensured that for each group of children, the vendor maintains a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
Upon arrival of the inspector at 10:00 AM, there were eleven children in care in the back classroom. When staff were asked for the attendance record, staff responded that the program director kept the attendance record. No attendance record was available for the group of children or for the groups when the children were split into two separate groupings.

Plan of Correction: The facility responded with the following:
Staff will be required to keep a current attendance record of the children in each grouping as it occurs (as children arrive and depart from each grouping).

Standard #: 22VAC40-665-540-A
Description: Based upon review of five staff records, the facility has not ensured that staff have been evaluated by a health care professional to be free of tuberculosis prior to interaction with the children.
Evidence:
The record provided for staff 4 did not include documentation that the staff person was evaluated to be free of tuberculosis. Staff 4 was on duty with the children during today's inspection and stated that she had been working with the children for "about a week",

Plan of Correction: The facility responded with the following:
Results of a tuberculosis screening will be obtained for within 30 days prior to employ for new staff. A new employee will not come into contact with children until the results of the screening have been obtained.
