An unannounced subsidy health and safety inspection was initiated on 01/24/2020 and completed on 01/27/2020 in response to a complaint that was received regarding documentation of children's injuries. Based on the information gathered which included staff interviews and record review, this complaint is found to be valid.
The violation is listed on the violation notice and was reviewed with the program director at the exit interview.

Standard #: 22VAC40-665-550-2 Description: Based on record review and interview, the vendor failed to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.
Evidence:
1. Child 1 received an injury while in care on 11/27/2019. This injury required outside medication attention.

2. Child 1 received an injury while in care on 01/14/2020. This injury required outside medication attention.
a. This injury was not reported to the department's representative.
3. The program director, staff 1, stated that she had not reported either injury to the department's representative.Plan of Correction: Effective immediately following any child injury resulting in the need for medical attention will be reported to our licensing representative. Director will review the standard will all members of management no later than 02/14/2020.
