Comments:
An unannounced monitoring visit and facility tour was conducted from 2:15 pm-3:00 pm. At the time of inspectors' arrival there were 56 school aged children in care with 5 staff members. Children were observed participating in free play, snack, departure and transitioning between activities. 5 children's records and 5 on site staff records were reviewed. Injury reports were also reviewed. No medications are being administered at this time. An additional record review was conducted at the Administrative office was on 2/14/2020. Areas of noncompliance are detailed in the violation report and were discussed with the Site Director at the conclusion of this inspection during the exit meeting.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review and interview it was determined the center did not ensure each staff member submitted documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment or volunteering and shall have been completed within 12 months prior to or 21 days after employing.
Evidence:
1. The record for staff #4(hired 12/23/18) did not contain documentation of a negative tuberculosis screening.
2. Staff confirmed there was no TB documentation for staff #4.

Plan of Correction: -

Description: Based on record review an interview it was determined the center did not ensure that each staff record contained all of the required documents.
Evidence:
1. In staff #4's record there was no job title and no references.
2. Staff confirmed staff #4's record was missing documentation.

Standard #: 22VAC40-185-90--A
Description: Based on record review it was determined the center did not ensure that the required written agreements between the parent and the center were in the child's rcord by the first day of attendance.
Evidence:
The record for child #5(enrolled 9/3/19) did not contain the required written parental agreements.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and interview, it was determined that the licensee did not deny continued employment of a staff who did not have a search of the central registry finding within 30 days of employment.

Evidence:
1. The record for staff #4 (date of hire 12/23/18) did not contain documentation of a completed search of the central registry finding.
2. Staff confirmed that the search of the central registry finding was not in the record for staff #4.
