An unannounced subsidy health and safety inspection was conducted on 02/10/2020 from 3:25 pm - 5:00 pm. A monitoring inspection was concurrently conducted as this program is a licensed child day center.
At the time of entrance there were 55 school age children in care with four staff. Children were observed working on home work in the cafeteria. Outdoor play and indoor table games additionally reviewed.
Records reviewed for 11 children in care.
Records were reviewed for 4 staff at the YMCA Greenbrier North Branch on 02/13/2020.

Standard #: 22VAC40-665-540-A Description: Based on record review, the vendor failed to ensure that the TB documentation shall have been completed within the last 30 calendar days of the date of employment.
Evidence:
1. The TB screening for staff 1 was completed on 06/07/2019.
2. Staff 1 stated that she began working at the site in mid January 2020.
a. The employment file for staff 1 indicates a hire date of 05/28/2019 and a separation date of 08/26/2019.Plan of Correction: TB tests were sent out. This will be corrected within 3 days of test.
Standard #: 22VAC40-665-580-B Description: Based on record review, the vendor failed to ensure that within the first 90 days of employment or subsidy vendor approval all staff who work directly with children shall complete Virginia Preservice Training for Child Care Staff.
Evidence:
1. There is no documentation of Virginia Preservice Training for staff 2.
a. Staff 2 began employment in September 2019.
2. There is no documentation of Virginia Preservice Training for staff 3.
a. Staff 3 began employment on 09/04/2019.Plan of Correction: Staff 2 no longer works for us. Staff 3 will and is working on pre-service
Standard #: 22VAC40-665-580-E-1 Description: Based on record review, the vendor failed to ensure that all staff have current certification in cardiopulmonary resuscitation (CPR) appropriate to the age of children in care within 90 days of the date of employment.
Evidence:
1. Staff 2 lacks documentation of current CPR certification.
a. Staff 2 has been employed since September 2019.

a. Staff 3 has a hire date of 09/04/2019.Plan of Correction: Staff 3 will do first aid & CPR training 2/26/20.
