A Subsidy Health and Safety inspection was conducted on 2/12/2020 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Violations were found during the supplemental subsidy health and safety inspection requirements and are listed on the violation notice.

Standard #: 22VAC40-665-540-A Description: Based on a review of six staff records the facility failed to ensure that all staff have a TB test or screening before coming into contact with children.
Evidence:
1. Staff #1 was hired 1-6-20 and had a TB screening on 1-9-20.
2. Staff #2 was hired 11-4-19 and had a TB screening on 1-14-20.Plan of Correction: The program director will ensure that all staff have a TB test or screening before coming into contact with children.
Standard #: 22VAC40-665-770-B-5 Description: Based on a review of policies and procedures for emergencies, the facility failed to complete all required plans.
