Comments:
An unannounced renewal inspection was conducted today from approximately 12pm-2pm. There were 13 children in the direct care and supervision of 2 staff. A sample of 5 children?s records, 2 staff records, 5 minor injury logs and 3 medications were reviewed. The children were observed eating lunch, washing hands, singing and napping. There will be a meeting with staff at the school tomorrow. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, A child's record was missing required information.

Evidence:
Child B was missing the address of 1 emergency contact.

Plan of Correction: Staff stated the parent will fix it today.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place.

Evidence:
Cleaning and disinfecting sprays were found in a unlocked cabinet.

Plan of Correction: Staff stated that they let the administration know about the required lock and they have not received an answer yet.

Standard #: 22VAC40-185-510-J
Description: Based on observation, all medication was not kept in a locked place.

Evidence:
The medication was located in a high cubby.

Plan of Correction: Staff stated I did not know I will get a locked box.

Standard #: 22VAC40-185-510-L
Description: Based on record review, the center failed to keep a record of medication given to children with all the required information.

Evidence:
1. The center did not have a medication administration log.
2. Medication given to Child A on 11/07/2019 was recorded on the back of the medication authorization form.
3. The following required information was not noted: any adverse reactions and any medication error.

Plan of Correction: Staff stated I will use it next time.

Standard #: 22VAC40-185-550-M
Description: Based on record review, minor injury logs were missing required information.

Evidence:
Of 5 minor injury logs reviewed the following was missing:
1. 1 was missing type of injury.
2. 1 was missing person present during injury.
3. 1 was missing future plan to prevent injury.
4. 1 was missing type of treatment .

Plan of Correction: Staff stated I will remember to fill in the entire sheet.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review, results for an out of state Central Registry Search were not received ad a follow up communication was not documented.

Evidence:
An out of state Central Registry Search was sent in November for Staff A. The results were not in and a follow up was not documented 45 days after the request.

Plan of Correction: Staff stated I will pick up the results from the office.
