An unannounced monitoring inspection was conducted between the hours of 10:00 am and 1:00 pm. Upon arrival there 63 children with 15 staff present. A sample of 6 children and 3 staff records were reviewed in addition to 1 medications. Children were observed eating lunch, during circle time and being read a story. Infants were observed being fed, rocked and sleeping.

Standard #: 22VAC40-185-260-B Description: Based on a record review the center did not have documentation of annual approval from the health department.
Evidence:
The last documented health inspection of the center was dated 07/25/2019. The director confirmed this was the only documentation that the center has.Plan of Correction: The following corrective action was provided by the director during the exit interview:
The health department had been out, however said they were coming back and did not leave paperwork. The director will call the health department today to follow-up.
