Standard #: 22VAC40-185-70-A Description: Based on review of eight new staff files, the facility failed to have all documentation required when obtaining phone references for two employees prior to employment. Phone references are to include the name of the person contacted, date of contact, firm contacted, and the results. Evidence: the phone references for Staff 5 and Staff 8 did not include documentation of the results. One reference check for Staff 1 was a relative by blood. Plan of Correction: All phone references will be documented with the results obtained as to character and reputation as well as competency. References from relatives will not be accepted.
