Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted on Friday, February 7, 2020, from 10:15am until 1:35pm. The census for today?s inspection consisted of 30 children. During the inspection, children were observed making valentine boxes, doing sticker art, playing with toys, and in circle time learning from flashcards. Children?s equipment, learning materials, outdoor and indoor space, restrooms, first aid kits and nonmedical emergency supplies and injury reports were reviewed. Five children?s records, five staff records, annual fire and health inspections, liability insurance policy, fire drill, lockdown, shelter-in-place drill logs, daily attendance logs, staff training and development activities, and all required postings were reviewed. Transportation is provided and the vehicles and transportation documentation were reviewed. Medication is administered and medication, authorization forms, and allergy care plans were reviewed.

The owner/director was available for the inspection. The owner/director was present at the exit interview, at which time, inspection findings were reviewed.

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations of the supplemental subsidy health and safety requirements were found and are identified on the violation notice.

Please complete the ?plan of correction? and the ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1.) steps to correct the noncompliance with the standard(s), 2.) measures to prevent the noncompliance from occurring again; and 3.) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations:
Standard #: 22VAC40-665-520-B-11
Description: Based on record review and interview on 02/07/2020, the provider did not obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: 1. The record of Child #1 (DOE: 09/09/19) did not contain documentation of an allergy care plan.
2. During interview, the administrator reported a written allergy care plan has not been obtained for Child #1.

Plan of Correction: Per the Center: "PLAN OF CORRECTION: Prior to the inspection, the director provided the child's father a form to be filled out by the child's physician.
DATE TO BE CORRECTED: The director has communicated with the father about the importance of having this documentation at the center. Documentation will be present no later than March 6th or the child will not be able to continue attending before/after school."

Standard #: 22VAC40-665-580-B
Description: Based on record review on 02/07/2020, the provider did not ensure within the first 90 days of employment staff who work directly with children shall complete Virginia Pre-Service Training for Childcare Staff.

Evidence: The record of Staff #4 (DOH: 9/17/19) contained documentation of Virginia Pre-Service training dated 2/6/20, which exceeds 90 days from the date of hire.

Plan of Correction: Per the Center: "PLAN OF CORRECTION: The director will be more aggressive in ensuring new staff complete the Virginia Pre-Service Training withing 90 days of employment.
DATE TO BE CORRECTED: The staff member completed the training on 2/6/20. This was prior to the inspection on 2/7/20."
