Comments:
A Subsidy Health and Safety inspection was conducted on Tuesday, February 18, 2020 to review supplemental health and safety requirements. The inspector arrived at 10:45am and concluded the inspection at approximately 12:47pm. The census for today?s inspection consisted of 10 children in the direct care of 4 staff members. Administrative staff were present and available to the children in care. The center?s director and assistant director were present and assisted the inspector throughout the inspection. Upon the inspector?s arrival, the children were engaged in a variety of activities. The inspector observed and inspected each classroom and completed formal observations throughout. The children were observed in their designated classrooms. The children in the 3s and 4s classroom were observed during a teacher-led activity. The children in the 2s classroom were observed preparing for nap time. The school age children were not present during the inspection. The required postings were observed and found to be in compliance. All areas where the children receive care, inside and outside, were inspected. The center?s first aid kit and non-medical emergency supplies were inspected and found complete. Medication is not administered at this time. Five children?s records, five staff?s records, and officers? backgrounds checks were reviewed today.

Fire Inspection: 06/20/2019 Health Inspection: 12/16/2019

A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. A violation was found during the supplemental subsidy health and safety inspection and is identified on the violation notice.

Violations:
Standard #: 22VAC40-665-540-A
Description: Based on a review of five staff records on 02/18/2020, the vendor did not ensure one staff was evaluated by a health professional and was issued a statement that the individual was determined to be free of communicable tuberculosis (TB) within the required time frame.

Evidence: 1) The TB results in the record for Staff #2, hired on 12/30/19, were dated 11/16/19. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Plan of Correction: Moving forward, the results will not exceed 30 calendar days prior to hire.
