Discussed electrical outlet in the hallway and office , injury record and training requirements.
An unannounced monitoring inspection was conducted on February 25, 2020, from 10:55 am -1:10 pm. Upon arrival to the facility, there were 5 children present with two staff members. The licensing inspector reviewed supervision, staff qualifications and training, nutrition, required posting, the physical plant and administration. The center is not administering medication at this time. The children were freely playing, eating lunch and table activity. Exit interview conducted with the Program Director.
Standard #: 22VAC40-185-140-A Description: Based on record review, the licensee did not ensure that each child had a physical examination.
Child #5 ( start date 8/6/2018) did not have a physical examination with in 30 day of start date.
Plan of Correction: Will try to locate physical or request a new physical.
Standard #: 22VAC40-185-70-A Description: Based on 2 of 5 records reviewed, the licensee did not ensure that each staff record was documented with all required information.
Staff #2 was missing two references.
Plan of Correction: The references will be completed .
Standard #: 22VAC40-185-260-A Description: Based on record review and staff interview, the licensee did not ensure a annual fire inspection was conducted annually.
Evidence: The most recent fire inspection was completed 2/20/2019.
Plan of Correction: Director will contact the fire inspection.
