An unannounced monitoring inspection was conducted between the hours of 10:00 am and 2:00 pm. A sample of 6 staff and 4 children's records were reviewed. Children were observed during nap. Infants were observed in cribs, being fed bottles and in bouncy chairs. Staff were observed on the floor with the infants.

Standard #: 22VAC40-185-280-B Description: Based on observation the center did not ensure hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the infant room there was a bottle of correction fluid sitting on the ledge of the window sill.
2. In the 2 year-old room #4, there was a contained of disinfectant wipes and bottle of bleach water on the changing table and in the bathroom there was a container of disinfectant wipes on a shelf in the bathroom.Plan of Correction: The following corrective action were provided by the director during the exit interview:
The director will remind staff that all chemicals are to be placed in a locked place. The director will follow-up to ensure future compliance.
Standard #: 22VAC40-185-280-G Description: Based on observation the center did not ensure substitute containers were labeled indicating its contents.
Evidence:
In the 2 year-old room #4 there was a clear spray bottle containing a liquid substance in the bathroom, in reach of children. The bottle was not labeled indicating its contents.Plan of Correction: The following corrective action were provided by the director during the exit interview:
The director will talk to staff about hazardous substances, how to label and store.
Standard #: 22VAC40-185-440-B Description: Based on observation the center did not ensure cribs were used by a specific child.
Evidence:
In the infant room there was a diaper bag in child #4's crib. Staff stated the bag did not belong to child #4, but was Child #5's bag, however, it was stored in child #4 crib due to lack of space.Plan of Correction: The following corrective action were provided by the director during the exit interview:
The director will talk with infant staff about crib use and and how to store items belonging to specific babies in areas other than the cribs.
Standard #: 22VAC40-185-500-C Description: Based on observation and interview the center did not ensure all requirements were met when using a toilet chair.
Evidence:
In the 2 year-old room #4, there was a small toilet that looked like a toy, with a yellow liquid substance in it. With the director present, the staff in the room confirmed it was actually a toy toilet chair being used as a real one and that the yellow substance was urine.Plan of Correction: The following corrective action were provided by the director during the exit interview:The toy toilet chair will be removed from the classroom today. If toilet chairs are used in the future, they will be emptied immediately.
Standard #: 22VAC40-185-520-C Description: Based on observation the center did ensure all requirements were met when using diaper cream.
Evidence:
In the 2 year-old room #4, there was a tube of diaper cream stored in the bathroom inside a cubby, that was in reach of children. The center did not have written permission by a parent to administer the diaper cream to the child, whose name was on the tube.Plan of Correction: The following corrective action were provided by the director during the exit interview: The director instructed the staff during the inspection to bring the diaper cream to the front office to have it sent home to the parent.
